2008 Medical and Liability Release Form

This form must be notarized before the student can leave for any trip with Crestmont Student Ministry.  Luckily, this form will last a calendar year and you will not have to fill one out every trip.  Please make sure you have one of these on file prior to the date of departure.

Hey Parents,

    I want to personally invite you to our "Parenting for Purity" seminar on September 21st, 2008 at 6:00 until 7:30 in the Dungeon. We will give tips for parents on how they can steer their teenager into purity. We as parents have a vital role to play in this area, so I hope you will come to this free, and informative seminar.  Pizza will be served.
